NEW PATIENT INFO
The practice is near full capacity therefore New Patient appointments are limited. *
~New Patient appointments are 1 hour with the Doctor so there is no same day availability.
~Our sincere apologies for any inconvenience this may cause.
~If we are unable to meet your needs, we hope you are able to find another
office that can accommodate you.
We do not want anyone to suffer any unnecessary pain or delay in their care.
~ Back In Motion does provide insurance billing ~
Please call the office to provide your information so we can verify your policy's coverage and benefits.
If you do not have insurance or it does not cover your care here, your first visit is $100 out of pocket and is a one hour appointment with the doctor which includes the treatment.
The office does not take xrays. If you have xray and/or MRI reports please provide them.
The doctors are in network (or participate with) Medicare and Cigna and are Preferred Providers with Blue Cross Blue Shield.
They accept and bill other insurances as out of network providers.
Back In Motion accepts cash, checks, MasterCard and Visa, including HSA cards.
They can also provide you with receipts/detailed statements for your Flexible Spending accounts. (These will be emailed to you after your date of service).
(We are unable to accommodate New Patient appointments on Saturdays)
*Arizona State Law defines a New Patient as someone who has never been treated by the Dr. or your last visit has been 3 years or more.
The Doctors are scheduling their own new patients appointments. If she is able to schedule you as a new patient, they will email you the new patient paperwork for printing and filling out before your appointment.
They will also call you a few days beforehand to confirm this appointment.
If you are unable to confirm or keep your appointment, they will give that appointment to someone on the waiting list. This provides the Dr.'s the opportunity to help another patient with their health care needs.
